What One-On-One Physical Therapy Actually Means At Pinnacle

In most insurance-based clinics in Lubbock, a physical therapist is scheduled across two to four patients at once. You get fifteen minutes of hands-on time, then a technician walks you through exercises while the therapist starts the next evaluation. That model exists because reimbursement rates require volume, not because it produces better outcomes.

Your evaluation, your manual therapy, your loading progression, and your reassessment are all performed by the same licensed DPT, in the same hour, every visit. Nobody hands you a printed sheet and walks away. The practical difference shows up in two places: your therapist notices the compensation pattern that a technician would not be trained to catch, and your program is adjusted the day something stops working rather than at your next re-evaluation three weeks later.

Pain is a symptom with a location, not a diagnosis. A patient who arrives with a shoulder impingement often has a thoracic spine that has stopped rotating, or a scapula that never learned to upwardly rotate under load. Treat only the shoulder and the pain comes back in four months.

Every Pinnacle plan of care starts with a full movement assessment: joint-by-joint mobility, motor control under load, and the demands of the sport or job you are returning to. We test, we treat, and then we re-test in the same session so you can see what changed. The goal is not “cleared.” The goal is that the tissue tolerates more load at discharge than it did before the injury — because the tissue failing under load is what caused the injury in the first place.

Your First Visit

What Happens At Your First Physical Therapy Visit

Your first appointment runs a full 60 minutes and follows the same structure every time.

  1. History

    What happened, what makes it worse, what you have already tried, and what specific activity you want back.

  2. Objective testing

    Range of motion, strength, joint mobility, and movement screening — measured and written down so we can compare at week three.

  3. Hands-on treatment on day one

    Manual therapy, dry needling where indicated, and the first loading exercises. You are treated at the evaluation, not scheduled to come back for treatment later.

  4. Your plan

    How many visits we expect, what we are progressing toward, and the home program — usually two to four exercises, not fifteen.

  5. A written copy

    The plan is yours to keep whether you continue with us or not.

Most patients are seen once or twice weekly for four to eight weeks, though post-surgical timelines run longer and are dictated by the surgeon's protocol and tissue healing.

Methods

Dry Needling, Manual Therapy, And What We Actually Use

Pinnacle DPTs use trigger point dry needling, joint mobilization and manipulation, soft tissue and instrument-assisted work, cupping, blood flow restriction training, and graded exercise loading. None of these is the treatment on its own.

Manual therapy opens a window in which tissue tolerates more range and less guarding; loading in that window is what makes the change stick. A clinic that only does hands-on work leaves you dependent on the table, and a clinic that only hands you exercises leaves you fighting a movement restriction you cannot get past. The value is in the sequencing, which is a clinical judgment your DPT makes in the room, session by session.

Why Cash-Pay Physical Therapy Costs Less Than Patients Expect

Pinnacle does not bill insurance directly. That surprises people until they run the arithmetic on a high-deductible plan. With a $3,000 deductible, an insurance-based clinic bills roughly $150–$250 per visit against that deductible and typically schedules two to three visits per week for six weeks — 12 to 18 visits, most of them paid entirely out of pocket until the deductible clears, most of them mostly technician time.

At Pinnacle you pay a known price per hour of your therapist's time, and you generally need fewer visits because each one is a full hour of skilled care. We provide superbills for out-of-network reimbursement, and we accept HSA and FSA cards. No authorizations, no visit caps, no surprise denial six weeks in.

Texas Direct Access

Do I Need A Physician Referral For Physical Therapy In Texas?

No. Texas has direct access, so you can be evaluated and treated by a physical therapist without a physician referral. Texas law does place a limit on treatment duration under direct access: a licensed PT may treat for up to 10 consecutive business days without a referral, and a PT who holds a doctoral degree or a residency/fellowship certification with the required continuing competence may treat for up to 15 consecutive business days.

Beyond that window, continued treatment requires a referral from a physician, chiropractor, dentist, podiatrist, PA, or advanced practice nurse. If you need one, we will tell you before you hit the limit and coordinate with your provider directly.

How much does physical therapy cost in Lubbock without insurance?
At Pinnacle, physical therapy is cash-based, so the price is known before you book. There are no facility fees, no separate charges for dry needling or manual therapy, and no surprise bills. How many physical therapy visits will I need?
Most non-surgical patients at Pinnacle complete care in four to eight weeks, seen once or twice weekly — roughly 6 to 12 visits. Post-surgical rehabilitation runs longer and is governed by tissue healing and your surgeon's protocol; an ACL reconstruction, for example, is a nine to twelve month process with the formal PT phase concentrated in the first four to six months. Because we are cash-based, nothing artificially extends or truncates your plan of care. There is no authorization to renew and no visit cap to spend down. Your DPT gives you an expected visit range at the first appointment and revises it at each reassessment, so you always know where the finish line is.
Do you treat people who are not athletes?
Yes, and they make up a large share of the caseload. Pinnacle treats adults with low back pain, shoulder pain, arthritis-related stiffness, plantar fasciitis, post-surgical joint replacements, and general loss of mobility — alongside competitive athletes. The performance framing is about method, not clientele: we measure what you can do, load the tissue progressively, and re-test. That applies identically to a collegiate pitcher and to a 68-year-old who wants to garden for two hours without paying for it the next day. If you have a body and a goal for it, the approach works. No athletic background, current fitness level, or sports history is required to be seen here.
Can I come to Pinnacle if I have already tried physical therapy elsewhere?
Yes, and a meaningful number of patients arrive that way. The most common reason previous therapy did not hold is that the treated site was not the causal site — the shoulder was treated while the thoracic spine and scapular control were left alone, or the knee was treated while hip strength and ankle mobility were never assessed. Bring your prior notes, imaging, and home exercise program if you have them. Your Pinnacle DPT will repeat the objective testing from scratch rather than inheriting someone else's assumptions, and will tell you directly at the evaluation if the previous plan was sound and simply needed more time.
What should I wear and bring to my first appointment?
Wear clothing you can move and be assessed in — athletic shorts for any lower body complaint, and a tank top or a shirt that can be moved for a shoulder, neck, or back complaint. Bring any imaging reports, surgical notes, or a previous home exercise program, plus your HSA or FSA card if you plan to use it. No physician referral is needed to start. Arrive a few minutes early for intake paperwork, or complete it in the patient portal beforehand. Plan on the full hour: the first visit includes evaluation and treatment, so you will leave with hands-on work done and exercises already prescribed.
