Peptide Therapy — Individually Prescribed, Provider-Supervised

Peptide therapy uses short chains of amino acids that act as signaling molecules to influence specific biological processes such as tissue repair, metabolic regulation, and growth hormone release. At Pinnacle Performance & Wellness in Lubbock, Texas, peptide protocols are prescribed individually by Stuart Riggins, FNP-C, after evaluation and lab work. Many peptides are compounded and prescribed off-label, and candidacy is a clinical decision — not a menu selection.

The Basics

What Peptides Are, In Plain Terms

A peptide is a short chain of amino acids — shorter than a protein, longer than a single amino acid. Your body already makes thousands of them, and several are drugs you have heard of: insulin is a peptide, and so is semaglutide. What distinguishes therapeutic peptides is specificity. Rather than broadly altering a system, a peptide binds a particular receptor and nudges one signaling pathway.

That specificity is the appeal and also the limit. A peptide that acts on one pathway will not fix a problem driven by a different one, which is why Pinnacle runs labs and a full history before prescribing rather than selling stacks. It is also why the honest answer about many peptides is that the human evidence is thinner than the marketing. We would rather tell you that up front than sell you a protocol you did not need.

Regulation

How Peptide Therapy Is Regulated, And Why That Matters To You

This is the part most clinic websites skip.

Some peptides are FDA-approved drugs with substantial human trial data — semaglutide and tirzepatide, for instance. Many others discussed in performance and longevity settings are not FDA-approved for the uses described, are prepared by compounding pharmacies, and are prescribed off-label. In 2023 the FDA placed a number of peptides, including BPC-157, into Category 2 of its bulk-substances review — substances with significant safety risks identified for compounding use — which restricts what compounding pharmacies may legally prepare. Availability for some compounds is therefore limited or has changed since you last read about them online.

Two practical consequences. First, anyone selling you “research chemical” peptides online without a prescription is operating outside that framework entirely, with no verification of what is in the vial. Second, a clinic that will prescribe you anything you ask for by name is not doing clinical work. At Pinnacle, what can be prescribed is determined by your evaluation, current regulatory status, and what a licensed compounding pharmacy can legally supply.

About these therapies. Peptides such as BPC-157 and TB-500, growth hormone secretagogues, and IGF-related protocols are not FDA-approved for the uses described on this page, and some are prepared by compounding pharmacies rather than manufactured under FDA approval. Off-label and compounded therapies may have less published safety and efficacy data than approved products. All prescribing decisions are made by a licensed provider after evaluation, and therapy is monitored. Individual results vary and are not guaranteed.

Scope

The Categories We Evaluate

CategoryWhat it targetsRegulatory status
Metabolic / GLP-1 (semaglutide, tirzepatide)Appetite regulation, glucose handling, body compositionFDA-approved for specific indications; off-label outside them. See Medical Weight Loss.
Tissue repair (e.g. BPC-157, TB-500)Soft tissue and connective tissue repair signalingNot FDA-approved for these uses. Compounding is restricted; availability varies.
Growth hormone secretagoguesEndogenous growth hormone release, sleep, recoveryNot FDA-approved for performance, body composition, or anti-aging use.
Other signaling peptidesImmune, cognitive, and longevity pathwaysVaries by compound; most are not FDA-approved for these uses.

Which of these — if any — is appropriate for you depends on your labs, your history, your medications, and your goals. Some patients are better served by fixing a hormone deficiency, a sleep problem, or a training and nutrition gap first, and are told so.

The Process

How A Peptide Consultation Works

  1. Evaluation

Full medical history, current medications and supplements, training load, injury history, sleep, and specific goals. In person in Lubbock or by telehealth.

  2. Labs

Metabolic panel, hormone panel where relevant, inflammatory markers, and anything your history indicates. Peptides are not prescribed to an unmeasured system.

  3. A frank conversation about evidence

What has human trial data behind it, what has animal data only, and what is mechanistically plausible but unproven. You should know which category your protocol sits in.

  4. Prescription and administration training

Where indicated, dispensed through a licensed compounding pharmacy, with injection technique, storage, and dosing schedule taught directly.

  5. Follow-up

Scheduled reassessment with repeat labs where appropriate. Protocols are time-limited by design, not open-ended subscriptions.

Candidacy

Who This Is Not For

Peptide therapy is not prescribed during pregnancy or breastfeeding, to minors, or to patients with an active malignancy or a history of certain cancers, given that several of these compounds act on growth and repair signaling. It is not prescribed to patients seeking a specific compound they read about online without an evaluation, and it is not an alternative to treating an underlying diagnosis.

Competitive athletes need to know that several of these substances — including growth hormone secretagogues and BPC-157 — appear on the WADA Prohibited List and on NCAA banned-substance categories. If you are drug tested by any sanctioning body, tell us at the consultation before anything is prescribed.

Common Questions

Peptide Therapy FAQ

Is peptide therapy FDA-approved?
It depends entirely on the peptide. Some are FDA-approved drugs with large human trial datasets — semaglutide and tirzepatide are peptides, and so is insulin. Many peptides discussed in performance and longevity settings are not FDA-approved for those uses; they are prepared by compounding pharmacies and prescribed off-label. In 2023 the FDA placed several of these, including BPC-157, into Category 2 of its bulk drug substances review, which restricts what compounding pharmacies may legally prepare, so availability has changed and continues to change. Any clinic presenting the whole category as FDA-approved is misinforming you. At Pinnacle, the regulatory status of whatever you are considering is stated plainly during the consultation, before you decide.
Can I just buy peptides online instead?
You can, and it is a genuinely bad idea. Products sold online as “research chemicals, not for human consumption” sit outside pharmacy regulation entirely — there is no verification of identity, purity, sterility, or dose, and independent testing of that market has repeatedly found mislabeled contents, contamination, and wide dose variance. You are injecting an unverified substance from an unaccountable supplier. Beyond product risk, there is no evaluation: nobody has checked whether your symptoms are actually caused by something a peptide addresses, whether it interacts with your medications, or whether you have a contraindication such as an active malignancy. Prescription peptides at Pinnacle are dispensed by licensed compounding pharmacies, with dosing and monitoring by a licensed provider.
Will peptides make me fail a drug test?
Possibly, and this needs to be settled before you start rather than after. Growth hormone secretagogues, BPC-157, and several other peptides appear on the World Anti-Doping Agency Prohibited List, and NCAA banned-substance categories cover peptide hormones and growth factors broadly. If you compete under any sanctioning body — NCAA, USADA, a professional league, tactical or military testing, or your employer's program — tell us at the consultation. We will either identify a compliant approach or tell you directly that there is not one for your situation. GLP-1 medications for a documented medical indication generally sit in a different category from performance-oriented compounds, but the specifics depend on your sport's rules and any therapeutic use exemption process.
How long does a peptide protocol last?
Most protocols at Pinnacle are time-limited, commonly running eight to twelve weeks with a defined reassessment point rather than continuing indefinitely. There are two reasons. Clinically, several of these compounds are used to support a specific repair or adaptation window — once that window closes, continuing does not add benefit. Practically, the long-term safety data for continuous use of most non-approved peptides does not exist, and prescribing as though it does would be dishonest. At reassessment we look at symptoms, function, and repeat labs, then decide whether to stop, pause, or continue. A clinic that puts you on a permanent recurring peptide subscription without a reassessment schedule is running a billing model, not a treatment plan.
Can peptide therapy be managed by telehealth?
Yes. Pinnacle provides peptide consultations and ongoing management by telehealth for patients in Texas, Oklahoma, Colorado, Kansas, New Mexico, and Arizona. The evaluation, review of labs, prescribing decision, administration training, and follow-ups are handled on video with the same provider each time. Labs are drawn at a facility near you. Medication ships directly from the licensed compounding pharmacy with instructions for storage and reconstitution, and injection technique is taught on the visit. Remote patients follow the same monitoring schedule as in-person patients — the delivery channel changes, the clinical standard does not.
Related care under the same roof: GLP-1 protocols live in Medical Weight Loss. Many patients evaluated for peptides turn out to have a treatable hormone deficiency — see Hormone Optimization. For localized tissue injury, PRP is evaluated alongside or instead of peptides, and recovery protocols work best alongside loading — the PT floor is in the same building.
